## Introduction A form is established to obtain the opinion of all employees on a well-defined subject. ## Manager Only the manager has the possibility to set up a survey by filling in all the presentation fields and adding as many questions as necessary.  For each question the manager can choose between 4 presentation modes: * CheckBox * DropDown * RadioButton * TextBox. By clicking on the pen, he can edit the list of possible answers.  ## User manual All other users can respond and record their responses. Saving the answers sends a feedback to the manager who can in turn download the answers in a .csv format.
